Leave Request Form Template

This leave request form template is designed for backend systems that generate PDF leave applications from structured JSON data. It includes employee details, a leave balance table, request dates, and approval signatures in a consistent layout. Use it for HR portals, internal admin tools, and workforce management systems where leave requests must be generated, stored, and signed as PDFs. The table layout is stable for server-side rendering and remains readable in black and white. Sections are separated to paginate cleanly for longer notes and policies, and signature fields are placed after the request summary to avoid overlap in multi-page output.
